South Country Health Alliance (SCHA) was formed in 2001 to improve the way people receive health care.  SCHA is now serving over 14,000 members in 14 Minnesota counties (including Freeborn County), offering three distinct programs to meet the needs of individuals of all ages and abilities.

                                                SeniorCare Complete

                                                Ability Care

 

The Care Coordinator promotes service accessibility and provides comprehensive coordination of all services to meet the needs of SCHA members across the continuum; social services, public health, medical and other community services.  The Care Coordinator works collaboratively with SCHA to provide support at the local level to meet SCHA’s mission, “Provide a member focused, community-based health care system.”
