South Country Health Alliance (SCHA) is a county based health plan that was formed in 2001 to improve the way people receive health care. SCHA serves over 33,000 members in 12 Minnesota Counties, including Freeborn County.
There are three distinct programs offered in Freeborn County to meet the needs of individuals of various ages and abilities:
The care coordinator works collaboratively with SCHA to implement a care coordination model that incorporates both medical and social services with Public Health and other community services. This enables SCHA members to receive services in a comprehensive and cohesive manner at the local level.
