Home and Community-Based Services (HCBS) are Medicaid programs that enable individuals to receive care in their homes or communities, rather than in institutions.

These services have a significant impact in the following ways:

  • Improves quality of life by enabling individuals to stay in familiar surroundings.
  • Is cost-effective, these services cost less than institutional care.
  • Increased autonomy allows individuals additional control over their daily routines and care decisions.
  • Increased community integration is encouraged through participation in work, education, and social activities.


MnCHOICES is the required first step to access Minnesota's HCBS programs. It's a comprehensive, person-centered interview that determines what services you qualify for and helps you plan to live independently. This assessment is the gateway to programs such as the Elderly Waiver and the Alternative Care program.
