About Your WIC Appointment

Freeborn County WIC is providing services by phone instead of in person. Please call (507)377-5289. You can continue to use your eWIC card in stores.
What to Bring
For every WIC appointment, please bring your WIC Identification (ID) folder.
For certification / recertification appointments, please be sure bring the following:
- Participant Proof of Identification (please bring one of the following)
- WIC Identification (ID) folder - if the participant’s name is listed on it
- If you do not have a WIC ID folder (please bring one of the following)
- Birth certificate
- Checkbook
- Crib card
- Driver's license
- Health benefits card
- Passport
- Pay stub
- Photo ID
- School ID
- Social security card
- Social services benefit card
- State ID
- Voter registration card
- Participant Proof of Address (please bring one of the following)
- Bill
- Checkbook
- Current driver's license
- Lease / Rental agreement / Statement from landlord / Rent receipt
- Mail addressed to the applicant
- Pay stub
- State ID
- Verification of Income Eligibility (please bring one of the following)
- Proof of participation in one of the following:
- Supplemental Nutrition Assistance Program (SNAP) (Food Stamps)
- Free / Reduced School Meals
- Fuel Assistance Program
- Head Start
- Medical Assistance / Minnesota Care
- Minnesota Family Investment Program (MFIP)
- Supplemental Security Income (SSI)
- Proof of all household income:
- Recent pay stub(s) for each person for each job, or
- Current tax return
- Proof of participation in one of the following:
Unable to Keep a WIC Appointment?
- Please remember that the person you selected to serve as a WIC proxy can bring children for their WIC appointments, if you are not able to do so yourself.
- If you are unable to keep the scheduled appointment, please call the WIC office, at 507-377-5289 (Spanish 507-377-5286) Monday through Friday, 8 a.m. to 5 p.m. as soon as possible, so we may reschedule the appointment for you.
